When a card fails in one reader, suspect the reader. When it fails in a camera, a laptop, a phone adapter and a friend's machine, stop suspecting anything external — the common factor is the card. Specifically it is the controller, the small processor moulded into the card alongside the flash, which handles wear levelling, remaps failing blocks and keeps its own private map of where each piece of data physically lives.
